Some people make a difference quietly. Others bring a whole neighbourhood together. The 5FOLD Community Awards are a chance to recognise the contributions that deserve to be seen.

Sports Person Talent

Recognising sporting achievement, positive conduct and an example that encourages others.

Youth Community Champion

Celebrating a young person whose leadership and service are making a difference.

Community Initiative

Recognising a resident whose idea addresses local needs and creates a meaningful benefit.

Most Valued Volunteer

Acknowledging sustained effort, reliability and care through volunteering.

Over 50’s Inspiration

Celebrating a resident aged 50 or older whose achievements and continuing contribution inspire others.

Prepare your nomination

Help us understand their impact.

You do not need polished language. A clear account of what the person has done is a helpful place to start.

  1. Choose the best fit

    Read the category descriptions and consider which one most closely matches the person's contribution.

  2. Be specific

    Describe what they did, who it helped and why it mattered. A few concrete examples will make their contribution easier to understand.

  3. Review before sending

    Check names and contact details, read your explanation once more, then submit through the official form.

A little recognition matters

Tell the story behind the effort.

Community contribution comes in many forms, from a long-standing commitment to a new idea that brings people together.

When writing your nomination, focus on the person's actions. Explain the difference you have seen and what makes their contribution stand out to you.

Keep the nomination relevant and respectful. Avoid including private information that is not needed to explain their contribution.

What should I prepare?

Have the relevant names and contact information ready, choose a category and prepare a short explanation of the person's contribution. Follow the labels and requirements on the form.

Read the complete criteria first, then contact 5FOLD if you still need help choosing.

Is an exact closing time listed?

The published deadline says close of business. Please ask the team if you need to confirm the cutoff before submitting.
